Updating address, phone number or email

You can request a change of address  by utilizing one of the four following ways:

* Please note we cannot accept mailing address change requests over the phone.

Phone number and email address changes can be made by logging into your CommunityPro® account and updating the information by clicking "My Profile" or by contacting CommunityCare via email at communitycare@sentrymgt.com or by phone 800.932.6636.
